    For most medicines in Britain, there is one obvious door: a GP appointment, a prescription and a trip to the chemist. Mounjaro, the weekly weight-management injection containing tirzepatide, is unusual in that two distinct doors have opened at once. One leads through the NHS, where access is being introduced in phases according to clinical criteria. The other leads through private, regulated online pharmacies, where eligible adults complete a consultation and pay for the medicine themselves. This feature sets out how the two routes differ, and, just as importantly, what stays exactly the same whichever door a patient uses.

    Two doors to the same medicine

    It helps to start with what does not change. Mounjaro is a prescription-only medicine, licensed by the UK medicines regulator and supplied as a pre-filled KwikPen. Whether a patient receives it on the NHS or privately, it is the same product, held under the same cold-chain requirements, with the same patient information leaflet and the same list of possible side effects.     What differs is who assesses the patient, how long that takes, who pays and how the ongoing relationship works. Those differences are significant enough that many people are weighing them carefully before deciding which route suits their circumstances.

    The NHS route: phased, criteria-based and specialist-led

    The NHS has approached weight-management injections cautiously and deliberately. Rather than making the medicine available to every adult who meets the licensed eligibility, health authorities have introduced access in stages, prioritising people with the greatest clinical need first. In practice, that has meant higher BMI thresholds and the presence of several weight-related health conditions for the earliest groups, with the intention of widening the criteria over a period of years as capacity allows.

    The strength of the NHS pathway is the wraparound care.     • Breadth of support: NHS programmes are designed around structured lifestyle support. Private pharmacies vary; some offer dietary guidance and regular check-ins, others provide the medicine with clinical oversight and little else. Patients should ask what is included.
    • Continuity: NHS treatment is reviewed within the service. Private treatment requires the patient to complete periodic consultation renewals so that the prescriber can confirm the medicine remains appropriate.

    The similarities matter as much as the differences, because they are the safeguards that protect patients.

    Fourth, the regulator is the same.     The emergence of a private, regulated pathway alongside the NHS has given eligible adults in the UK a genuine choice about how they access Mounjaro. The NHS offers structured, funded care within a phased system; private online pharmacies offer faster, patient-funded access after an online consultation. What both share is the thing that matters most: a prescriber’s decision, a pharmacist’s oversight and a regulated supply chain that patients can verify for themselves. Whichever door a person chooses, those safeguards should be the first thing they look for.
